Appointments and Scheduling: The Take 2 Nutrition Consultant, Carol
Eshleman, will come to your home first for a one-hour nutritional and
health assessment to determine your ideal size and review all health
issues that may relate to your diet. You may choose at this point to
schedule either 4, 8, or 12 weekly one-hour nutrition counseling
sessions, and you are certainly welcome to schedule additional sessions
once your initial sessions are completed. Appointment times are MWF
9:30 - 10:30am and 1:00 - 2:00pm.

Should you need to cancel or reschedule an appointment, please call in
advance.
*If your home is not in Eastbank Jefferson or Orleans Parish in Louisiana, you may
meet Carol at her home in Metairie. Address and directions will be
emailed as needed.*

Nutritional Assessment - $55
Nutrition Counseling
4 weeks - $200
8 weeks - $360
12 weeks- $480
*The total balance for your counseling sessions is due in cash or check at your first session.*
